Privacy Statement LEA SCORE by LEA AXIS Research

LEA SCORE located at Dapperstraat 55-1 1093 BP in Amsterdam, Holland, is responsible for the

processing of personal data as described in this statement.

LEA AXIS Research is developer and supplier of LEA SCORE that allows users to obtain a risk

assessment of their personal psycho-metabolic lifestyle through a personal environment. LEA

SCORE is offered through an application, website and dashboard. By using the app, you agree

to this Privacy Statement without limitation.

During the processing of personal data LEA AXIS Research works conform the requirements of

the applicable data protection legislation, like the General Data Protection Regulation. This

means we:

- clearly specify our purposes before we process personal data, by using this privacy

statement;

- limit our collection of personal data to only the personal data needed for legitimate

purposes;

- first ask for explicit permission to process your personal data in cases where your

permission is required;

- take appropriate security measures to protect your personal data and we demand the

same from parties who process personal data on our behalf;

          respect your right to access, correct or delete your personal data held by us.


If you create an account within LEA SCORE and thus gain access to your own Personal

Environment (PO), personal data will be collected and processed. LEA AXIS Research is the

party responsible for all data processing. In this privacy statement, we will explain what kind of

personal data we collect and for which purposes within our online service LEA Score. We

recommend that you read it carefully.

If you have any questions regarding the processing of personal data, you can find the

contactdetails of LEA AXIS Research at the end of this privacy statement.

As soon as you fill your LEA SCORE environment with your lifestyle data, we automatically store

it encrypted and process it for you. We work on the basis of the principle of “privacy by design”.

You can make your stored data transparent for healthcare providers and other third parties and

also (temporarily) share it with them. This is only possible after you have given explicit

permission for this.

We respect the privacy of LEA SCORE users and ensure that the personal information you

provide to us is treated confidentially. For this reason, the personal data collected by us is

carefully processed and secured.


Registration:

Certain features of our service require you to register beforehand. You will have to provide some

information about yourself and choose a username and password for the account that we will set

up for you.

We process personal data whereby a distinction is made between the personal data that is

processed for your account and the lifestyle data that you store yourself via LEA SCORE. Your

personal data will also be used to send our newsletter. For this we use the following data:

- E-mail

- First name

- Surname


When creating a LEA SCORE account, you leave certain information with us, including personal

data. We only collect, store and use this personal data that you provide directly.

The following personal data are processed to create and manage a LEA SCORE account:


  • First name
  • Surname
  • E-mail
  • Age
  • Sex
  • Country of birth
  • Education
  • Working hours
  • Body weight Height


We have a legitimate interest in doing this. We store this information until you close your

account. We will retain this data so that you do not have to re-enter it every time you visit our

website, and in order to contact you in connection with the execution of the agreement, invoicing

and payment, and to provide an overview of the products and services you have purchased from

us.

The following personal data is processed for the purpose of storing and sharing your lifestyle

data: Gender, age, country of birth, education level/working hours per week and work status,

body weight and height, personal lifestyle data at a psychological level, medical history, eating

and social behaviour. Basically, data that becomes available through your use of LEA SCORE's

system. This data will be encrypted before it is stored.


For internal scientific research:

LEA SCORE -for its internal research- will process certain data that the user shares about his

lifestyle. This data is anonymized and then ends up in Microsoft Excel. The anonymized Excel

file is then processed with SPSS in which the data is numbered 1 to 5.

The purpose of this is to use this data anonymously for psychometric and statistical research

within LEA AXIS Research to gain a better understanding of patterns of diabetes, depression,

cardiovascular disease & lifestyle.

In order to be able to share data with each other, an export must be made of the information. You

can share information via, for example, Whatsapp or Gmail. Or via the LEA SCORE dashboard

function.


Purposes of the data processing:

We process your personal data for the following purpose: execution of the agreement.

Using LEA SCORE, your answers can help you understand how your own lifestyle may be

impacting your psychological and/or metabolic health. You create your own psycho-metabolic

profile, and you can optionally share it with others.

An account is created for the use of LEA SCORE.

You can (after your explicit permission) give other third parties, including, for example, your

(health)care providers (temporary) access to your data or share your data with them by using

WhatsApp or Gmail, or via the LEA SCORE dashboard option.

LEA SCORE is not responsible for the communication that a data subject maintains via the

above-mentioned social media. For information about the processing of your data via WhatsApp

or Gmail, we refer to the privacy statements of WhatsApp or Gmail, or via the LEA SCORE

dashboard option.

LEA SCORE processes your lifestyle data for its own scientific research. This data is only used

for scientific research in the public interest.

If you give your explicit permission to receive the newsletter, your personal data will also be

processed to keep you informed of developments within LEA SCORE.


Cookies:

We use functional cookies at LEA SCORE to enable you to log in and to personalize LEA

SCORE. A cookie is a small text file that is placed on your hard drive or mobile device by a web

page server.

In addition, third parties place cookies on the LEA SCORE website/app through the use of social

media buttons. Your consent is required for the use of these tracking cookies. You will be asked

for this permission before the cookies are placed. These buttons make it possible to easily share

your LEA SCORE account with third parties.

You have the option to accept or decline cookies. Most web browsers automatically accept

cookies, but you can usually change your browser setting to disable some or all cookies. If you

choose to disable all cookies, LEA SCORE may not be usable.


Grounds of the data processing:

We process your personal data on the following basis(s):

- You have given permission for the processing of your personal data. We process your personal

data for creating your account and sending the newsletter.

- We have a legitimate interest in processing your data: LEA AXIS Research. For this we use

your complete profile data, anonymised.

- Processing is necessary for the performance of our services. In order to carry out our services,

it is necessary that we process the lifestyle data you have entered. This data is encrypted before

it is stored.


Confidentiality and disclosure to third parties:

For the execution of our services, the personal data is stored with the following hosting provider:

We may transfer your personal data to countries outside the European Economic Area (EEA).

This happens, for example, because service providers we work with carry out their activities from

those countries. If necessary, we take appropriate safeguards to protect your privacy. This

means, for example, that we use standard contractual clauses approved by the European

Commission to make agreements about the handling of your personal data.

Your medical data can be viewed by the healthcare providers if you have given permission for

this via LEA SCORE. For example, you can receive a link to LEA Score from your GP. With your

explicit permission, your GP will receive your profile if you choose to send this to your doctor via,

for example, Gmail, or via the LEA SCORE dashboard optiion The care providers themselves are

responsible for the lawful processing of this personal data. This responsibility does not rest with

LEA SCORE. In addition, the medical data can also be shared with other third parties who have

their own LEA SCORE account if you have specifically given permission for this.

When you are not a direct care provider and you manage LEA SCORE user data via the LEA

SCORE dashboard option, you are only authorized to manage this user data after data

anonymization. Data anonymization means deleting or encrypting so-called identifiers that

connect a person to stored data.


Third Party Links:

LEA SCORE may contain links to websites operated by third parties. We have no control over

the privacy policies or practices of such third party sites and you should review the privacy

policies and terms of use of those sites for more information about the policies that apply to those

sites.


Retention periods:

We do not store your data longer than necessary to achieve the purposes stated in this privacy

statement. You can delete and export your data yourself, you can request us to have your LEA

SCORE account deleted personal data via email. Then we manually remove your data from the

database.


Security of your personal data:

Only the necessary persons have access to your data. Furthermore, access to your personal

data is protected and our security measures will be regularly tested based on a security

assessment by a legal hacker. In addition, we use SSL certificates for the secure transfer of your

data and communication.


Your rights:

If you have provided us with personal data, you can send us a request to view, change, move or

delete this data. You have the following rights:

- Right of access to your personal data: you have the right to obtain an overview of the personal

data that we process about you.

- Right to correction of your personal data: if we process incorrect or incomplete personal data

about you, you have the right to correct or supplement the relevant personal data.

- Right to restriction of the processing of your personal data and right to object: in some cases

you can request that the processing of your personal data (temporarily or otherwise) be

restricted, for example if you doubt the accuracy of the personal data or if you believe that the

processing is unlawful.

- Right to erasure of your personal data: in some cases you have the right to have your personal

data deleted, for example if the personal data is no longer necessary or you believe that the

processing is unlawful.

- Right to data portability.

- Right to lodge a complaint with a supervisory authority.

- Right to withdraw your given consent.

- Right not to be subject to automatic individual decision-making including profiling unless

expressly consented.

- The aforementioned rights can only be exercised if it concerns the personal data that are

processed for your account. With regard to the encrypted medical data that is not visible to us,

you can export and delete this data yourself via LEA SCORE.

You can submit a request to exercise your rights by contacting us by sending an email to

info@leascore.com.

At your request, we may ask you to identify yourself. We request data for this to ensure that you

are the correct person to whom the personal data belongs.

Ideally, we will comply with your request within 30 days. However, this period may be extended

for reasons related to the specific rights of data subjects or the complexity of the request. If we

extend this term, we will inform you in good time.


To file a complaint:

If you feel that we are not helping you in the right way, you have the right to lodge a complaint

with the privacy regulator. For the Netherlands, this is called the Dutch Data Protection

Authority.


Changes to this privacy statement:

We reserve the right to modify this statement. We recommend that you consult this statement on

a regular basis, so that you remain informed of any changes.
